Dues:

Please pay by a check made out to “Lae`ula O Kai" or with cash.  Take your payment to Missy Dunham or Sharon Balidoy or mail to:

Adult = $75 ($78 paypal)
Youth = $40 
Recreational = $45  ($48 payapl)

You can pay with a credit card or Paypal. 
It costs a little extra because we have to pay fees

Your dues pay for

1. your regatta racing fees,
2. a club tee shirt,
3. insurance fee,
4. and MCHCA card fee

Recreational paddler = $45

Waivers:

Print, fill out and bring a new 2010 waiver to Sharon:

Adults and minors are on the same waiver now.  Print out the HCRA 2010 Waiver on white paper. Be sure the parent or guardian signs a minor waiver for anyone under 18 years old. Also don't forget to initial each page to say you read both pages.

 

New to Lae'ula and/or Paddling?

Please either bring, email, fax or mail Sharon a copy of your identification to verify your birth date and name:

1. Driver's license - or
2. Birth certificate - or
3. Passport - or
4. Another document that verifies your name and DOB

Get it to Sharon by either:

1. Bringing it to practice - or
2. it to Sharon - or
3. fax it to 242-1576 - or
4. mail it to Sharon Balidoy, 114 Kono pl. Kahului, Hi 96732

Transfers

If you paddled with another club and are coming over to Lae'ula this year, you'll need to fill out a transfer form.
